Texas Parent to Parent provides support, information, and education for families of children and adults with disabilities, chronic and mental health conditions and other health care needs. The organization uses a parent-to-parent peer support model to help families navigate the challenges of caring for children with special health care needs. Services: Peer support; Parent-to-parent support; Resource referral; Education; Medical Home Toolkit; Care Notebook; Advocacy training; Pathways to Adulthood; Weekly Parent Support Group; Peer Parent Mentor Program.
